Well, you might indeed need all these packages but they are irrelevant to the problem at hand. Did you check the minimal example i posted just a few minutes ago? The solution to both your problems is given there. You should start to keep your eyes open. Please start with reading my signature.
Right after that, go back to the mwe i posted and click on »open in writelatex«
